Home Visits
We provide a mobile service which means we can visit your home, workplace or see you in the community to provide assessment, intervention and therapy to help you reach your goals.
These include:

NDIS assessment and intervention
We provide assessment and intervention for NDIS participants to help you meet you goals.

- Assessment, intervention and reporting for:
- Meeting access to the scheme
- Functional capacity assessments for change of situation or capacity building intervention
- SIL and SDA assessment & reports
- Home modifications assessment, recommendations and reports
- Assistive technology assessment, prescription and reports

Home Modifications
Being independent and feeling safe in your living space is an important part of life. We are able you to help maintain your independence in your own home.

We can provide recommendation for minor or major modifications to home environments such as;
- Ramp to access the house
- Grab rails to make your bathroom safe and comfortable
- Modifications to allow increased circulation space for a wheelchair or walking aid.
- Education and training on modifications
- And more!

Cognitive Assessment and Therapy
Cognitive assessment and intervention can be provided for people who may be noticing changes in their memory, difficulty concentrating or struggling to complete simple every day tasks.

We can work with you, your doctor / health care provider and/or family to detect early cognitive changes and provide you with strategies and support to manage every day tasks.

This can include;
- Cognitive and memory assessments
- Cognitive rehabilitation with personalised recommendations
- Education and training to you and your loved ones or carers
- Comprehensive cognitive assessment and functional cognitive reports

Assistive technology prescription
Assistive technology (AT) helps enable a person to simplify everyday tasks.
AT provides the opportunity for people to live and perform every day activities independently, access the community and engage in meaningful tasks.
Our team at MTS love seeing our clients achieve their goals.
Assistive technology assessment & prescription may include recommendations for:

Bathroom equipment to help you safely access the bathroom, toilet and wet areas of your home

Adaptive equipment such as modified cutlery, dressing sticks, clothing donners

Hospital beds, pressure cushions or mattresses

Smart technology e.g. voice to text, ipads, home monitoring systems

Specialist Reporting
MTS offers a wide range of reporting services to our clients and providers.

This includes;
- NDIS reports i.e. access request support documentation, FCA, SIL and SDA applications
- Home assessments and recommendations for reports
- Reports required for assistive technology and equipment recommendations
- Work cover and return to work documentation (case by case basis)
